The 2016-2017 Kindergarten Registration for South Park Elementary Center has begun. In order for your child to be eligible for Kindergarten, the student must reach the age of five (5) before September 1, 2016.
Registration packets are available to pick up at South Park Elementary Center between 8:00am and 3:00pm Monday through Thursday.
Time is running out to be registered for the 2016-2017 school year.
Your child must have the following immunizations before entering school:
4 doses of DTP (1 dose must be after the 4th birthday)
3 doses of the Polio vaccine
2 doses of MMR (1st dose must be after the 1st birthday)
3 doses of the Hepatitis B vaccine
2 doses of the Varicella Vaccine (1st dose must be after the first birthday) or a written statement from a physician indicating month and year of disease or serologic proof of immunity.
All students entering school will also be required to have a physical and dental exam dated within one year of entering school. If you are going to the doctor prior to registration you can find the medical forms for proof of physical/dental exam on our web site at www.sparksd.org and click on Nurses/Health Services. You will also need a birth certificate for registration.
In August we will have our annual Kindergarten Bus Orientation. This is about a 1 hour event where every kindergartener who attends gets to visit the school and also ride a school bus around our community. This event will be held on Wednesday August 24 at 9:30 AM at the Elementary Center Door #2.
